Community Health Workers Hub

COVID-19 Vaccine Toolkits

As a Community Health Worker, you are a crucial resource for your community. You have already guided many people through difficult health decisions and helped them cut through the clutter of COVID-19 information. Your sacrifice, dedication and commitment has not gone unnoticed.

The PHMC team recognized a gap in COVID-19 resources available to CHWs in our region. That’s why our teams created this project to help support you in your work to address COVID-19 vaccine myths and concerns of your community members.

Below is a comprehensive and easy-to-read COVID-19 toolkit that covers a variety of topics addressing vaccine hesitancy, links for additional resources and free trainings available to you.

Community Health Worker (CHW) Certification Training Program

The National Nurse-Led Care Consortium (NNCC) is offering a FREE, online training program for community health workers (CHWs) to learn strategies, tools, and resources to advance health equity, particularly in light of the lessons learned from the ongoing COVID-19 pandemic. Our 17-week training program will provide you with the skills to excel in providing care coordination for underserved members of our communities, and lead you to becoming a PA-Certified CHW.

Through this series, we intend to create a space where we can facilitate supportive conversations and learning across the community engagement workforce. Each interactive session includes individual and small group workshops to practice key strategies.

AUDIENCE: Community Health Workers (CHWs) across Pennsylvania and Delaware.

DATES: New training dates are added as they are scheduled. Please check this page periodically for opportunities to join us.

Learning Collaborative for CBOs: Emergency Management and Preparedness

In collaboration with the National Nurse-Led Care Consortium (NNCC), the Healthcare Emergency Management team at Public Health Management Corporation (PHMC) will provide a 4-part learning program on the fundamentals of preparedness planning. The program will describe promising practices to promote a system that: protects the health and safety of staff, clients, and facilities; reduces future vulnerability to hazards; builds organizational capacity; and allows you to quickly assess and communicate risks within your respective communities. Our 4-part program is designed to help you improve your prevention, protection, and response strategies and provide you with tools to respond to public health emergencies, including that of COVID-19.

Learning Objectives

Upon completion of this learning collaborative, participants will have the opportunity to:

  1. Engage with community leaders and advocates in conducting a community-based planning process
  2. Design an implementation work plan unique to your team and programmatic activities, developed through an analysis of risk
  3. Identify and prioritize operational resource demands
  4. Integrate and synchronize efforts through the lens of sustainability
